By checking the "I agree" box below, the person completing this form agrees and acknowledges that (1) he or she is the person named as the applicant; (2) he or she has been given the option to print out a paper copy of the Agreement and sign it manually, has agreed to sign this Agreement using an electronic signature, and for purposes of authentication, validity, enforceability, admissibility and otherwise, such signature shall have the same force and affect as a manual signature; (3) this Agreement and the transactions contemplated hereby are not conditioned upon the agreement to conduct the transactions electronically; and (4) he or she may still be required to provide a traditional signature at a later date.
The Applicant further agrees and acknowledges that the tree(s) may be removed, pruned, or encroached upon, only according to the criteria established by Article 15-50 of the City Code, and that by signing this form, the Applicant agrees to the conditions of the permit. If trees are requested for removal, the Applicant’s signature certifies that they are located solely on his/her property. The Applicant authorizes the City Arborist or representative to visit the property to inspect trees.
The Applicant also understand and acknowledges that submittal of the application and payment does not constitute a permit. This application is pending approval. City staff will review the submitted application for compliance with City Code.
This document that you will submit will be an online application for a tree removal permit. If payment was paid at the time of submittal, or if the tree is dead or has fallen, this application will be processed. Payment must be received before the application can be processed unless the permit is to remove a dead tree (no live growth) or a fallen tree. In those cases the application fee and neighbor notification period are waived.
City arborist staff will inspect the tree(s) and notify neighbors that a permit for tree removal has been requested. Please allow a few days for trees to be inspected followed by a notification and appeal period of 15 days. You will receive the same notice that goes out to your neighbors. It has the deadline to appeal the permit request on the notice. If there are no appeals, a permit can be issued the day after the deadline passes.
For additional information or the status of the permit, contact Christina Fusco at 408. 868.1276 or via email at firstname.lastname@example.org
